What are your hours? 8 a.m. to 5 p.m., Monday thru Friday. Our office is closed from noon to 2 p.m. daily.
What is your latest appointment time? 4:30 p.m.
Do you accept walk-ins? Yes. We understand that it may be difficult to find a time in advance to visit our office and will do the best we can to accommodate your schedule.
What are your lab hours? 8 a.m. to 5 p.m., Monday thru Friday.

New Patients and Forms

What should I bring if I am a new patient? We will need photo identification and medical insurance card with name and number. Please click here to find forms you may download and fill out before your first visit.
What is the no-show policy? Yes. Please click here to be directed to our No-show Policy Form.
Can my spouse speak to you about my account? If you would like your spouse to access your financial records, we will need written consent by you. Please click here to be directed to our Notices of Privacy Form.

Prescriptions

Do I need an appointment to get a prescription refill? This will depend on the date of your last visit, the ailment treated during that visit and the prescription needing refilled. Please note that all narcotic refills, by state law, require a doctor’s visit and review.
What do I do if I run out of medications? Please contact our office at 470-452-4948 and the after hours doctor on call will contact you.
How long does it take for you to process my refill request? Refill requests are processed in the order they are received and will be ordered within 24 hours.

Billing Questions

When am I expected to pay? Payment is due at time of service, and payments may include (but are not limited to) deductibles, co-pays or any and all outstanding balances.
Who do I contact if I have a billing question? 470-452-4948. Please ask for our billing department.
What forms of payments do you accept? Visa, Master Card, American Express, cash and check with two forms of identification.

Insurance

What insurance do you take? We take all insurance providers EXCEPT Kaiser Permente at this time. We are working to be able to accept them and will keep you informed when this changes.

General

Are flu, pneumonia, shingles and other vaccinations available through your office? We commonly have all of these but the shingles vaccine. Please call for availability.
Do you have a translator on-site? We speak multiple languages at our office, but can have a language service in attendance if we have a patient in need. At this time, we provide in-office language services in English, Farsi, Dari, Pashto, Hindi, Gujarati, Swahili, Nepali, Russian, Spanish, Urdu and Arabic.

Physicians

What type of physicians do you have? Please click here to find out more about our attending staff.
If my regular doctor is unavailable, will I be assigned to someone else? You may see another attending physician or our nurse practitioner.

Your Lab Results

How long will it take to get my lab results back? Normally, one to two weeks, depending on the needs of the patient.
Can I get my lab results over the phone? To ensure you can receive your lab results over the telephone, you will need to complete the Notice of Privacy Practices Form. Please click here to find out more.

Your Medical Records

The patient-physician relationship is extremely confidential.  Any information exchanged is guided by HIPAA regulations and is held in the strictest of confidence.  Confidential medical information will be released only with the express consent and authorization of the patient.

How do I request my records? First Medical will need written consent by you. Please click here to be directed to our Notices of Privacy Form.
Is there a charge for my records? If you are seeking copies of your records for an attorney, they will be charged per page. If we are replicating records for another doctor, a specialist or a hospital, there is no charge.
How long does it take to get my records? One week for complete files, but same day if you are needing files from just one visit or test result. Please let us know at the time you order you files if you would like to receive them via US Mail, email or fax.
